When and where I grew up you took driver's ed as a summer class in school and got your license on your next birthday. You could take driver's Ed when you turn 15. I don't remember there being a test.

1 for car, 1 for motorcycle

Did it thrice. Operating a motorized vehicle caused me to freeze in terror periodically and I needed a lot of hours to overcome that.

Usually you make one slight fucking mistake or do something out of habit like only checking your mirrors when changing lanes that causes a lot of people to fail.
Also, the instructor is EVERYTHING. If you get paired up with a shitty, grumpy old fucking dude/girl then you're fucked no matter what.
