You know I hate to be the guy who's all "this only works if you're Chad!" but this only works if you're Chad. No girl is going to respond to a note like this. At best they'll just toss it. At worst they'll show their friends and laugh at you or everyone who works there will think you're a creep.

There's something especially low value and beta about bringing a pre-written note and handing it to the girl instead of just saying to her, "hey, I think you're cute, here's my number" and THEN writing it down.
