I have crazy realistic dreams all the time and in fact predictive dreams.

I have recorded a lot of cases where things have come true that are beyond self fulfilling prophecies, and the dreams that come true are insanely realistic.

Let's hope that's not the case for you.

Worst pain I ever felt was being impaled in a fever dream, was almost 15 years ago and I still remember it vividly.

When last time i had very high fever I was dreaming about wierd complicated 3D puzzles wchich felt like they were combined with wires and computers which i had to resolve.

I do. I've also had dreams where I died and didn't wake up and the dreams usually continued with afterlife stuff. Once after dying in a dream I dreamt of waking up in my bed and died again this time to zombies invading my room.

It is kinda like pressure. I can get similar feeling when I press my stomatch with my finger. It's more disturbing and intense when I sleep. like something sharp is knocking into my guts.
