The story as I understand it is he got some kind of cancer, I believe throat, from smoking. Instead of getting it treated by the doctor's recommendations he tried all kinds of 'alternative medicine' which obviously didn't work. The cancer then blew up his face and he got his face replaced by leg and ass skin.

So it's entirely his own fault for looking like a monster. Chad's DO live on easy mode, but their low intelligence can sometimes result in early game overs or permanent debuffs.

That's not what happened.
The guy got a very rare and aggressive type on skin cancer. He had the tumor removed but the doctor had to take more skin and bone than expected. He lived with an open wound for nearly a year until he heard about a different doctor and went to him. The new doctor is a self proclaimed "humanitarium" and proposed an op that would take skin from his leg and forearm to cover his wound and eventually make him look normal. He went through with the surgery, making him look like op's pic on July 7, 2017.

In short, you understood the story horribly. This guy got unlucky both in his condition (face cancer) and the op that removed his tumor.
