He's a mouth breather from an infant probably. He sucked his thumb a lot and always breathed with his mouth. And his face was developing his face grew downward instead of outward. It created a vertical profile, resulting in this deformity that affects every inch of the facial bones, muscles and teeth. He also has very narrow arches(width is very short both top and lower jaw) which is a growth deformity probably from mouth breathing as well.
