In short, you remove your balls. It's not really a tranny specific proccedure. People with cancer or that had some kind of accident (testicular torsion, balls smashed by something hitting them, etc).

For MtF the reason its' done is so your body produces less testosterone which means you won't need to take anti androgens which fuck with your liver on the long run.
