I've never given anyone headpats but in highschool my 10th grade there was a senior girl who would always give me headpats and big hugs just because she was nice. I'd be sitting there and suddenly I'd have her face really close to me and her arms around me. She had a bf but it always made me feel good and like I must not be disgusting if someone did it so often. It was an almost daily occurence.

I enjoy that too, and admittedly get off to it a bit, but I'm already small so, I always feel small and it's my biggest insecurity

You should do it only to a girl you know. The occasion is important. Like congratulating or for doing something she's proud of. Out of context can be patronising.
There's also cultural norms. Westerners will always take it as insubordination but it's somewhat acceptable and playful in Asian subcontinent where I live. I have patted girls on the head and pinched their cheeks when they do something mischievous. Read the situation carefully before doing anything.

We live too far apart to see each other often, but on rare occasions that we meet, my sister and I exchange headpats.

It is the most comfortable and warm feeling ever.

Not previous poster, but I've never had that reaction. Rather enjoy it quite a bit, whether it's my imouto or my aunt who does it. On rare occasions other relatives, but mostly those two.
