Nah, the "bag of worms" appearance comes about from something called a varicocele, which is when you have dilation of blood vessels in the scrotum. Lack of this finding doesn't exclude testicular torsion, but generally speaking the pain associated with that is incredibly excruciating. Maybe you have prostatitis or orchitis. Only the doctor is going to be able to properly diagnose you.

What happens is that some men have a testicle that isn't properly attached to part of the inner scrotal lining. When this happens, the testicle is prone to rotating. If the testicle rotates, the spermatic cord (the "tube" that connects the testicle to the rest of the urogenital system) twists as well, constricting blood vessels and causing a great deal of pain. Like, unfathomable amount of pain that brings grown men to tears. If you have this happen to you, you're gonna immediately call yourself an ambulance and get yourself to the hospital. It's awful. The fix is pretty simple, usually just involving reversal of the twisting.
