Not sure why the guy is saying you need to pay all of MGS to understand it. This joke really just comes from MGSV: The Phantom Pain.

MGS games always bend the concept of what a "game" is and MGSV is no different. This game introduces characters in weird ways such as this. The first time almost any character came on screen, a little bar exactly as you see in the meme would be at the bottom of the screen, saying the character's name and a pseudo mysterious tag line.

Ex. First Line - Soldier Guy
Second Line - A hero without a past
Ex 2. First Line - Sexy super soldier woman
Second Line - A warrior denied a country

In this meme, it's as if wojak was a character in MGSV, specifically the one who's name is Punished Snake, has an eyepatch, shards in his head and scars all over (not important). This meme is simply portraying what Wojak's title card would be if he was in the game. Don't over think it too much, it's really just a reference.

He takes up his role as big boss once again, but with permanent damage (piece of metal in head, no arm), hense the name "punished snake"

thanks man. i tried researching i even went to that knowyourmeme site for it, but it didnt explain it properly in the sense that would explain why so many other non-gaming characters get the punished name (ie trump, cosby, jesus). and i was also wondering what the heck is sticking out the right said of the head, looked like a horn, and why it wasnt taken out. thanks again. excuse if english is bad in my post.
