I believe Misaki suffers from BPD, or it's heavily implied throughout the manga. The fact that she'd throw away Satou's love away even though it's one of her greatest desires portrays one of the personality disorder's traits, impulsiveness. Not to mention her fucked up childhood which raised the likelihood of her rather strange behavior.
