No need for sterilizing because you'll hardly get a disease, but make sure you have a blade just for cutting. Also, don't go around dirty places with open wounds and don't let the blood dry if you get some dirt on it.

Most of the pain will come a bit after you cut so use that to your advantage; if you want a lot of pain cut a lot before the pain truly hits and if you don't want too much take it more slowly or you might be overwhelmed.
