It really depends on the photos, you pretty much have to get a photo that resembles the original one in position and skin color. The closer you get to that the easier the job. Still that is not a quick shot quality for me, that would take me around 30 to 40 to make it look like MrPoo's.

Thanks. I started a tutorial a while ago, but didnt have energy to finish it.
Basically its:
- Find matching pair of tits. Getting same perspective is key, color not so much. No hard shadows and preferably same kind of lighting conditions.
- Fit boobs to girl
- Match color by using a few different techniques
- Match lighting
- Match noise levels
- Make picture look nice as a whole

what tools do you use to match color/lighting? do you auto-blend the layers with the panoramic option? I've seen that in a few tuts. then an adjustment layer with layer mask to refine it?

Nah, man. Im pretty old school. Using the same techniques I used when I was into model photography a few years ago. I blend manually with a layer mask.

For color matching I mostly use the hue/saturation adjustment layer bound to the masked layer with the boobs on them. The work has to be done in the master/red/yellow and sometimes green channel. I like it since I can change both hue, saturation and lightness in the same pane. Usually I cant get everything done at once, so I might need another one for another part of the image or maybe finish up with a bound solid color layer in soft light or overlay blend mode at 5-8%.
Extra lighting/shade is added with curves and masking (check dodge and burn technique)

These are pretty nice dude. Never thought of using the hue/sat layer to match color. I usually just use curves. How do you match noise? Ive always wanted to do something like that and tried a few things but wasnt happy with how they looked.

- Make a new layer and clear it to 50% gray.
- Bind it either to the original or the donor (alt + click between the layers).
- Add noise from filter meny and try to match amount in the other picture.
- Set it to soft light blend mode.
- Scale the noise layer until the noise pattern have the same size as in the other picture
- Adjust opacity on noise layer until same as the other picture.

Sure. Here is the donor. Flip the picture horizontally, then rotate and scale to match.
I rotate and scale with the donor layer in like 30-40% opacity, then add an inverted layer mask and paint with white in the mask to reveal the boobs.
