My pick: Himekishi Lilia. Super high production values for animation and audio allow ambitiously creative sex scenes to have the believability required to be the most extreme, heart-wrenching, yet beautifully erotic hentai I have seen. Other hentais have done more "extreme" things (as written on paper), but they're not even a fraction of Lilia's traumatizing effect without its beautiful production.
