What adjustments do you play with when xraying? Levels, curves, Bri/Con, etc... I just can't get the nipples to pop out nicely as in sample.

This is as far as I'm getting. How do you keep that white-ish tone but make the nips pop in another color? Is it using different layers?

exactly
painting the skin tone over the tits and blend the layers
then using a different color for the nips and repeat
