The head and jaw angle is most important. Next is hair. Then there is coloring and shading. If the original has lots of varying colors or the shading changes across the pic, it is very hard to match with a donor to make it look good. Keep in mind that porn pics are done in a controlled lighting situation (usually) so odd lighting on an original makes it tough. Natural sunlight is also a problem in the original because it is hard to match.
