depends on the original picture and what you really want to do. if you're intermediate it'd take 20-30 mins to make a nice image (i may be wrong).
you'd have to use the filters for changing the colors first. then the brushes like healing brush tool or clone stamp tool to make the tits or the dick bigger. i usually combine all of them to achieve what i want as a result.
