Ah yes, us fellow oldfags on /qa/ right now....
A few years ago someone posted on /b/ a video of (allegedly) himself robbing a store. He was wearing a gopro and recorded everything for a total of around 5 minutes: around the end of the video he gets into a car and drives away. It was there that I noticed that the scene was taking place in Italy, my home country. I skimmed through the video looking for relevant clues and I ended up figuring out approximately where the robbery had taken place, and I posted an article from a local newspaper in response to the OP where there were pictures of the same store appearing in the video talking about the robbery (happened a few days before). Both the thread and the video were quickly deleted.
Another time, on /b/, someone were posting blurry pictures of a naked girl in bed saying she was her daughter and he was going to beat her and rape her. He said he was very drunk and by the way he typed it was most likely true. Thread gained momentum and eventually OP posted a vocaroo of the girl crying, then a few more pictures and then disappeared. He kept writing things like "she's going to taste daddy's cock" and "she deserves this" and so forth. I believe this was actually happening and was not an elaborate trolling.
I could tell many other weird things I have witnessed on this website, and for all I know they could all be fake. I will never know.

The worst thing I ever saw was the pics on /b/ of the Mexican girl cut into pieces by one the cartels and I'm pretty certain it was right after it happened.
