How do we fix /qa/?
we start by murdering you
It works perfectly fine there's no need to fix anything.
/qa/ has shed the chains built in by moot and is just starting to display it's own unique attitude. It doesn't need to be fixed, if anything the lax moderation here is old school and allows for a more aggressive board mentality which requires Anonymous to actually think more often.
