That's assuming the missile they launch contains a nuclear warhead or not. It would be hard to tell if it is nuclear while in flight, though size and trajectory could give us a good idea on where it's heading, and how long it's set to fly.
We'd shoot it down as a potential threat, but I don't really see us going full "glass the norks" mode until a nuke actually hits Guam.
Preemptive strikes would be a good deterrent against future attacks, but it's all a matter of how messy Trump wants to make the aftermath look. Killing hundreds of thousands of already starving and brainwashed North Koreans won't look good, but every attack has a price.

I think it is more likely there would be a long protracted war involving sanctions against China.

If there is an exchange of fire against between the USA and NK I highly doubt they will glass all of North Korea. Probably North Korea will be left 'disabled'. But the stalemate will remain because the USA/South Korea/Japan won't be allowed by China to finish off North Korea.

Trump won't be using any nukes this year. Maybe as the war continues he will do so in a couple of years.
