I think they'd have to be evacuated pre-emptively, at this point North Korea will be pretty fucked, as they'll lose their 'hostage' if they choose to strike first then they lose the moral high ground and any military response will be justified. If they allow the south to leave they will probably be forced to capitulate or face complete annihilation. Not ideal but in the long term I think it will save lives as the problem will only get worse if they choose to start sending icbms to capital cities around the world.

If korea really united and distanced itself from the west, there is almost nothing stopping china from grabbing the land itself.
