I wouldn't know about Singapore's system, but I do know that I broke my leg and shattered my ankle and got 2 operations. They cost around $20k each.
I stayed in the hospital and was looked after for about a month. All of that was free I didn't even have to think about paying a cent for these world class surgeries. When I got out of the hospital I was provided 80% of my wages while I was in a wheelchair and unable to work, all provided by the government. That was so I could support myself and live to the same standard as I would if I was working, until I was fit to work again.
Now I'm fit and working and providing for my community and country again.
There is nothing wrong with looking after your volk. The government helped me out big time and it also made me realise what socialist health care systems are like. Very good.
Now I'm not going to pretend like this system will last forever, it won't, especially with liberal policies allowing tons of third worlders, Asians and Indians into my country but we are still small. Just over 4 million, it's working for now, but as the population increases and the demographics change from majority white this system will not hold up.

I'm sitting in one of the top 3 spinal injury hospitals in the world right now. And every other patient is a nog or non-English speaking mud who immigrated here because their brother's cousin got a green card.

What happens is the syste has to create criteria to refuse the care OR to refuse payment to the care provider after the fact.

This is what we are experiencing now with Obamacare. If you don't meet specific metrics the ACOs ( Obamacare designated insurers ) will simply withhold payment for individuals or blanket reduce their payouts to you acrossed the board by X percent.

This is forcing care providers to start refusing certain patients due to the risk of non-payment.
