They bill at rediculously high rates because the billing system is based on billing insurance companies not individuals.The idea is that if they bill an enormous rate, as long as sometimes someone pays it, then it's worth it. Otherwise it's counted as a massive write off and generates a fake paper loss.

Imagine you bill 10 people a 1,000,000 bill for emergency surgery. Imagine 1 person can pay through an insurance company. The other 9 can't pay that, so they bleed them as much as they can for money. Let's say they manage to collect 100,000 from each person. So out of a 10 mil bill they collected 2. But guess what? Since they "incured" another 8 million they won't collect, they write it off as a business loss and offset their income.

Now they collected 2 million and have no official income. Welcome to the scam.

Well if I could get away with it, why not find a way to create a huge paper loss so I can say I didn't make any money.
