Spain under Franco was already aligned with Hitler. They just didn't join Axis because they just had a civil war and in recovery.

Attacking Italy meant going through Alpine ridge, which German armored division can't pass. Also Il Duce was immensely popular.

Even further Stalin started to exert pressure on Eastern Europe, and wanted to take Romanian oil fields. Germany blitzing through East would have made Stalin very uneasy.

USA did care. FDR was very anti-German and itching to declare war on Hitler.

Likely the outcome of the war if Hitler was victorious, was gonna be Europe in pretty bad shape for a few decades. If he managed to develop the nuke first and eventually discourage the United States of Israel to join the war, eventually the Jews would have been removed in the states as well. If Russia was nuked, they would eventually adopt a NatSoc government and prospered themselves.
If these highly unlikely events happened, the Western world, probably would have been a white man's dream, but unfortunately, today, the situation is far worse than maybe even Hitler thought. The Jews are winning and they will destroy this planet along with us all.
