because as you go into higher voltages, insulation cost, both in cables and post insulators will rise. sure power losses would be lower, but then shock hazards would be greater which kinda comes back to insulation costs. all in all i doubt either 230 or 120/240 is the most cost effective but both these voltages and freqs were mainly chosen to reduce cost and industrial protection from other countries, and the cost it would take to raise/lower the entire LV grid of a country like the US would probably be massive to the point that it's obvious this is a slide thread
