If I told you that aliens were speaking to me through my TV, you might examine me for paranoid schizophrenia.
If I had a paranoid phobia of germs everywhere I went, I should be examined for OCD.
If I decided that I should kill myself despite no clear problems, I should be treated for depression.

But if I have a persistent feeling that I should be the other gender (and that doing serious self harm is a good way to deal with this)...
