Here's how I see it playing out (but IANAG)

US damages
>North Korea launches nuclear strikes at Guam, Hawaii, San Fran, Seattle (some or most of these fail, but at least one hits and detonates)
>US missile defense is able to intercept some but not all of North Korea's missiles, probably saving San Fran and Seattle
>At a minimum Guam is seriously damaged by a nuke, Hawaii is also possible

North Korea wouldn't do this without being willing to fight a full scale war, so it's likely they also use nuclear weapons on US bases in Japan and Korea. North Korea has between 5 and 20 nukes.

For the US response, decapitation and destruction of any remaining nuclear arsenal is the immediate priority. I would go so far as to say several Minuteman missiles would be launched, because they have the quickest response time. Subsequently, the US would inform the Security Council that it was going to conduct a nuclear response. The ambassador to NATO would immediately activate Article 5, calling NATO into the conflict. US bombers from Guam and Hawaii may be disabled by the nuclear strike, so any air based nuclear response would require planes from the US mainland. These would be smaller weapons, targeting troop and artillery concentrations.

I don't think you understand how technologically primitive they are.

With the Norks, the use of nuclear weapons would be more of a strategic diversion, launched to distract the US/NATO from a massive conventional attack across the demilitarized zone into South Korea.
