The answer is difficult. It would be much easier to do it through the federal government of course, but Conservatives are strongly against any form of government intrudence in their lively hoods. So a system like say Taiwan's wouldn't work if it was left up to the states. For the states, you have to make it so insurance companies can compete to offer the best prices. Much like how car insurance works

Trump's moves wrt Kushner are the biggest question mark, to me, about Trump and his intentions. They are totally, 180 degrees at odds with all the spot-on "America First" populist rhetoric of the campaign.
