A poorly made phone charger still has a DC converter. Shitty cheap chinese versions don't care about safety and are not designed to be shorted, it can cause parts to melt even at low voltage since amps are high. This can cause a short on the voltage regulator next to the outlet.

You have to be pretty unlucky for this to happen, or it needs to be shorted long enough to cause a problem, like if the end fell in the water for several minutes while she fell asleep, that could do it.

You're too dumb to live if you think taking any wires from the outlet and sticking it into a bathtub is a good idea.

Also, with DC, it may not matter if it's on a circuit breaker or GFCI outlet, it only takes milliseconds for electric shock to be fatal. Some of those only help if it's an alternating current that's shocking someone. DC kills fast.

120v 20amp AC to 5v 0.15amp DC converters are notoriously cheap and break easily. When they do, usually it just stops working. In rare cases it starts a fire. In very rare cases it shorts out and the full 120v 20amp goes down the line and fries whatever device is attached. In even rarer cases a person is holding the device at the time and promptly dies. It's sad, but basically the answer is don't buy a cheap 99c charger made in china, get a higher quality, name brand one.
