It was probably a cheap chinese designed charger with a common "ground" connection between the AC side and the 5v DC. It has happened before. Short the 5v, the regulator shorts & fuses internally, 120AC gets dumped to the 5v output... Bam, fried 14 year old.

Never get in the bath with something plugged into a wall outlet, even if it is presumed to be a small DC voltage. It starts as much higher, and you would be betting that some underpaid chinese designer values your life.

Electrical engineer here, this sounds like the most convincing explanation. I don't know the exact design of the power supply vut the AC input was probably connected to the DC output by fault.

What still puzzles me is why the RCCB didn't work. These things are nromally designed to trip within milliseconds in case of an error current so things like electrocuting yourself in the bathtub is more like a meme from a movie or from times when these devices were slow as fuck. This should be investigated as well.
