That accident was entirely the pilots fault. Both pilots. The co-pilot should have spoke up about flying thru fog.

What happened in this video is a classic case of what we call spatial disorientation. The pilot appeared to be relying on visual cues to maintain his attitude while those cues were completely shrouded by poor visibility conditions. His body's innate sense of up and down, left and right became conflicted with what his eyes were telling him and thus the control inputs required to keep what he thought was level flight (it wasn't).
What happened to the pilot is quite common particularly in helicopter accidents in conditions such as these, low visibility and ceilings. Helicopters themselves as machines are, by their nature so maneuverable and sensitive to control inputs that achieving extreme attitudes is quite easy with any moderate movement of the flight controls.
End of the day, the pilot was grossly, criminally negligent in his decision to continue flight into conditions that restricted his ability to distinguish the visible horizon. Believe it or not, this happens frequently even to experienced pilots, but the solution here would have been for him to turn back before he entered those conditions and heeded the flight manual and safety notices for that particular helicopter which all scream at you to never, ever, enter those kinds of conditions in the first place. BUt he had a set of customers to satisfy and a wedding party to impress. We call it get-there-itis. Don't let it happen to you, even if you're piloting an automobile. Poor decision making can be deadly.
