Now that America is finally on its knees, it would be an ideal time for the glorious ANGLO nation to roll in and exact revenge on all of the recalcitrant colonials and their descendants who dared raise their arms in defiance against the Empire.

At first no one will care about control, local officials will be preoccupied with feeding their populations and maintaining order in the reality of massive food shortages caused by the nuclear winter. These food shortages will lead to people coming together in gangs of sorts to steal food and resources to survive. Any large city that wasn't hit by a nuke will be just as bad off as their streets turn into a war zone.

In time two main types of powers will emerge, firstly will be those gang leaders who amassed enough power to become a sort of local governmental force able to keep the peace, and gather the resources needed to maintain a community. Essentially tribal war lords and strongmen. The second class would be the remains of old institutions such as the militarily, police force and state governments reforming their broken institutions and coalescing enough force to maintain order in their territories. Given no outside interference, either the old institutions would eventually re-take the united states and we would see something reminiscent of todays governmental structure, or one of the warlords would win and we would have a fascist sort of system.
