I totally agree. One important step towards lower costs would be that if someone goes to the hospital without consulting a normal physician first, he/she would have to
a) come up for the entire treatment if it's a minor thing
b) pay a part of the treatment if the hospital visit is somewhat justified
c) pay nothing (full insurance coverage) if the visit is completely justified

You didn't do your research very well there. Every citizen needs to have basic health insurance and if you as a company provide that, you can't deny anybody coverage. Of course you also have additional health insurances (e.g. for care at home) and those can be denied by the company. Also, the prices of most services are set if you want the insurance to pay for it.
