Swiss government spending as a percentage of total GDP floats around the same area as Australia (33-36% of GDP per annum), and we're one of the most regulated places on earth and not at all a free market paradise that you want Switzerland to be. The reason why the government intervenes in economies as much as they do(even in the most freemarket economies) is because it has to.

a) come up for the entire treatment if it's a minor thing

You didn't do your research very well there. Every citizen needs to have basic health insurance and if you as a company provide that, you can't deny anybody coverage. Of course you also have additional health insurances (e.g. for care at home) and those can be denied by the company. Also, the prices of most services are set if you want the insurance to pay for it.
