It's both. Lazy eye and a droopy eyelid.
Surgery can fix the eyelid. Sometimes surgery can work on the eye itself, but sometimes not. See her brain has learned to ignore parts of the vision with conflicting information (suppression). So if you physically straighten her eyes by surgery, if her brain doesn't adapt to the new position by "turning on" the suppressed area and gaining binocular fusion, then it's likely the eye will just go back into the position that it is used to.
