Gender - My take

Now to gender-
1. Gender dysphoria probably has existed at some base level forever due to biological causes. Life was probably really shit for these people when no options for dealing with it were available. But even the solutions now available are no guarantee of happiness.
To call these people mentally ill might not be quite right. Or at least we need to distinguish between illnesses that can be cured, and those that can't. For example are autism, ADHD and "sociopathy" always illnesses, or just outlier modes of operation? This is one of those nuanced questions.
2. It is possible, indeed probable that some people construct their sense of alternate/mismatched gender. Or that their experiences/socialisation does this for them. So they end up at some point feeling a bit weird about their gender identity, and with encouragement of other individuals or the overall culture, decide to embrace their feelings as indicative that they must be transgendered. Essentially, they have talked themselves into (or allowed themselves to be talked into) believing they are trans. Some of these people might make drastic changes that may end up making their lives no better, or even worse.

Also, a corollary for you. Way back in the 80's Bi phobia was a thing in gay and lesbian (and straight) communities. So people that felt a bit homo were forced to choose, from both sides. No doubt some made a choice that they weren't comfortable with, or that wasn't best for them. But at least they weren't cutting parts of themselves off.
