A bunker system set up in such a way where you have 20+ floors of aerated metal between you and the surface will absorb all the shock of a powerful nuke easily. "Aerated steel", literally air-filled steel, being researched right now, could probably do the same with much tougher metals.
I'd hardly be surprised if the obscenely rich live in underground cities most of the time and only pop out every so often just to say hello. (or even pretend they did, using doubles or literally realtime CG which is very reliable now)
