What really needs to happen is that the pope should be elected and chosen (and be made to abdicate if necessary) by the pentarchy. Not for me, should be by council, and only for egregious errors.
As for the Pentarchy itself, in a reunited Church I would expand that to include all apostolic and patriarchal sees and those close enough to it, e.g. Rome+EC patriarchs and catholicoi+Carthage(restored) + Toledo + all EO+OO+ACoE patriarchs and catholicoi.

As for Constantinople I would demote it in the taxis to fifth due to neither being apostolic (the claim of St. Andrew founding it is fabricated) nor originally a metropolia (former suffragan of Heraclea)
