It's surprisingly easy to knock out the average person untrained in martial arts. Especially smaller persons/women. Also, the hit you get from the fist is actually not such a big part of the equation in the street - in many cases most of the injury occurs when the head hits the hard street.
People can easily get lasting brain injuries from it.
