In my experience in Texas and NY it was always very confusing, they ask how you are, you answer how you are and ask how that person is. that's how it works in Germany.
In the US whenever a stranger asked me that he either continued talking before i could answer or said it almost attached to the greeting, as i said, like it's just a greeting in the US while over here for example you tell that person how you are and what's been up.
