Was there an attempt to get a warrant for a wiretap on the Trump campaign? If so, why?

Was the warrant given?

Did the NSA or another US agency tap the trump campaign?

Did another state agency tap the trump campaign? And if so, did that agency pass off what they found to the NSA or other US agencies? Was it at the request of permission of the US or did it just happen anyways?

Also, who was in command of this operation. Did Obama know? Is the NSA going around tapping presidential campaigns on their own initiative? If trump was tapped by the NSA, I'm not sure which option bothers me more.
