Yellow filter tends to make skin look much smoother. Grain hides the bigger imperfections.

I don't see the problem though. That skin is fine, possibly because of pre-existing makeup.

clone stamp in photo shop, soft edges, 30%ish opacity, fade it away, and then a subtle sharpening tool if it starts to look too smooth
