I live in a no-inspections state, and it's almost always the shit heap behind the wheel that's a problem, not spontaneous mechanical failure.
If you're really worried about safety, you'd be better off inspecting the drivers. Inspecting the cars is just for pollution, taxes, and creating some do-nothing jobs.

Virginia here. Have an EFI to carb swapped 1997 302 in my 1989 Mustang with straight pipe exhaust. Passes inspection every time. All they really check at the place I go to is lights, brakes, and tires. They could give a fuck less about whats under the hood cause they drive around in their V8 honda shoebox killers as well.
