Inspections are only a pain for people who know what they're doing. They're for everyone else who wouldn't know a brake rotor if it fell out of the sky and hit them in the head, to help keep them from killing someone else when their car fails due to their negligence.

Thing is, inspection programs invariably become corrupt and end up doing nothing but lining the pockets of inspectors and repair shops.
As for why people don't maintain their cars they view it as an unwanted expense that has no immediately obvious ramifications.

at least in my shop in PA, it's the gubbiment more than anything, I don't get jack shit for doing inspections.
