You overestimate Japanese infrastructure. Ever been to rural Japan, or even a smaller city?

I can see people parking their cars once they get to Tokyo but that's about it.

I think concerns about manual driving being outlawed, or becoming prohibitively expensive due to insurance, don't make sense.

'Self driving' in the future will be set up so that the car prevents you from getting into trouble (basically a better version of existing self braking and steering tech). I.e. All the same sensors that'd be engaged with self driving will still be on and monitoring everything, and will take control away if you're doing something stupid.

Simply equipping all vehicles with transponders would be wonders for active crash avoidance. Cyclists and pedestrians could have transponders built into their cell-phones. So long as your old pre-self-driving car was retrofitted with a transponder the self driving cars would know exactly where it was.
