OP if you want a real answer, I remember reading an interview with Shuichi Shigeno. It was from around 2009 iirc, the interviewer was asking him a bunch of questions about the origin of the story and characters and stuff. When they were asking about the title, he said he came up with Initial A at first but couldn't think of anything cool that the A could stand for, so he changed it to Initial D where the D stood for Dingus.
