This is because you're either not shifting hard enough, or you're twisting the gas back on too hard. I do this on my SV sometimes wether I use the clutch or not. The only solution I've found so far is to slow down just a bit, and be more determined in my actions.

This one will never be guessed so I will give it to you for free.
It was my friend's W123 240D with a gearbox from an old sprinter (don't ask how that happened).
If you wanted 2nd you would always need to do the circular motion or it will grind the gears and go into R.
And 3rd gear always grinded for some reason.
1st and 4th were almost normal, just some play in the stick to get it to slide in the right place.
R and 5th were smooth as fuck.
