We can't really tell you, you just have to decide if it's worth it. If you really love driving a manual to the point that it outweighs the pain, then keep it up. But most people won't judge you for driving an auto because you have an injury
Cant you Drive a semi automatic? Shifting with paddles etc.
Buy a nice dct and save what's left of your knee.

I'm 21 and have early-onset RA. Unfortunately I had to give up manuals.
