I think I wrap my thumb and forefinger around it, with the middle finger poking down in to the boot. Kinda like holding a pencil, except the middle finger isn't grasping anything and just kinda resting against the shifter.

When driving large trucks, I put both forefinger and middle fingers over the front of the knob, and rest my thumb on the left side of the knob. Except when shifting from 5th-6th, I then grab it like I'm just grabbing a benis/pole/grenade and keep my thumb on the side, that way I can flick the splitter with my forefinger or middle finger.
