I did, two years ago. It was during an aerobatic competition, the pilot was supposed to do a loop with a single roll, but lost speed while he was at the top of the loop performing the roll, and fell into an inverted spin. He then probably panicked and kept pushing as if he as trying to exit an upright spin. Instant death with cremation. I knew the guy too.
