In more then 100 years of operation it never did. It had been heavily bombed during WWII but no car did catch fire.

In case of an accident there are several options.
- the car moves to the next station by itself
- the car is pushed to the next station
- a car goes alongside, passengers change cars and are brought to the next station
- evacuation with ladders
- passengers are abseiled like this

The concept is perfect for its city because of a number of special geographic conditions. Barmen and Elberfeld grew rapidly during the industrial revolution and had a ton of roads built for use by pedestrians and coaches. They were narrow and prone to congestion. All the important centers of trade and commerce were right next to the Wupper river and they still are. The city center is basically a 15km/9 mile long narrow strip straddling the river and the lowest leves of the valley. That means a single line with no crossings or switches was and still is enough. An underground metro could not have been built because of the rocky ground and terrain. Instead, the went for a suspension monorail that straddles the river.

suspended is more stable. Gravity pulls it down ontothe track, while a normal monorail gravity pulls it off the track. Just like a ball on a curved surface will remain still when it is in a valley, or when it is on the exact top. However in the valley moving it to the side makes it return to the central position, while on the top it will immediately roll off.

I believe generally suspended monorails have a smaller turning radius because they don't need much "track width" to stay stable. This is especially true for the Wuppertal schwebebahn as this is litterally a monorail as in they use a single normal railway rail as track. I visited wuppertal as a kid and I remember being pretty flabbergasted seeing a complete metro train turn in a 15 meter turning loop

Döppersberg facing west towards Ohligsmühle, you can kind of tell from the run of the river and the bridges, and the Schlossbleiche on the right side hasn't changed much. Also you can see the Thalia theater behind the buildings on the left, where the Sparkassenturm stands is now.
