He appears to be a mouthbreather (hence why his midface collapsed) and likely has had poor nutrition and lifestyle habits for bone health. Couple that with his hairline recession and his aesthetics have deteriorated. His neck also is too narrow (should be about equal to widest point of face in frontal view).

Note however that he wasn't actually particularly handsome in the original image, but it's clear how he has deteriorated.

My friend and I met a homeless guy at the train station once who told me I looked like the singer from Incubus. He bummed a cigarette from my friend and told us he had just gotten out of prison then gave me a sip of some Steel Reserve he had in a 7/11 cup. Cool guy.
